Vaccine dates
Type the dates from the patient's vaccine records. Tap Email to MA and staff receive a clean list plus a paste code. The MA pastes it into Part 10 of the I-693 in one step, so nothing has to be retyped.
What the MA receives
Edit an I-693
Open any I-693 made with this page, applicant or completed. Every part opens for editing, and nothing is filled in or changed for you. When you email it, staff get it marked Edited with a list of what changed.
Complete the medical sections
Open the PDF the applicant sent you. Their answers load below, then you fill Parts 6–10 and save one combined I-693. You can save a draft and reopen it later, for example when IGRA or lab results come back.
Vaccine records
For staff · optionalTake a photo of each page of the applicant's vaccine records, or choose files. They're emailed with the form so the MA can enter the dates. They are not added to the I-693.
